Student Health Insurance Waiver Information

Unity College requires all students enrolled in 6 or more credit hours to have health insurance coverage. Each academic year you will be automatically enrolled in the Unity College Student Health Insurance Plan. Students preferring to waive the Student Health Insurance Plan and charge from their tuition/fees billing, must be enrolled in a health insurance plan and are REQUIRED to complete the on-line waiver form. The on-line waiver form must be completed and submitted by January 24, 2014. The waiver process must be completed each academic year.

Students wishing to supplement their current insurance with the Unity College Student Health Insurance Plan may do so by NOT completing the online waiver process.

For those Unity College students who do NOT wish to be included in the Unity College Student Health Insurance Plan for the 2013-2014 academic year please login and complete the online waiver process. 

You must have the following health insurance information ready before you begin the online waiver process:

Policyholder Name

Insurance Company

Policy Number

You will have only ONE opportunity to complete this online form, so be certain to have your Health Insurance information before continuing!
